SCHOLARSHIP TERMS & CONDITIONS
This document outlines the Terms and Conditions for all scholarships offered by Colorado International Soccer Academy (CISA). Scholarship awards provided to athletes help them meet a portion of their player fees. You must read these Terms and Conditions in their entirety before you accept your scholarship offer. When you accept your scholarship you are agreeing to and accepting these Terms and Conditions and agree to the requirements and conditions of your scholarship.

Complete the Scholarship Application Form (one per person). All information requested must be provided. Incomplete forms will not be considered.

Scholarships may provide a percentage discount for families up to 50% of the registration cost. All applications will be reviewed in a case by case basis..
- Request for scholarships for the player registration fees must be submitted at least two weeks prior to the start of the fall or spring soccer season.
- Need is the primary criterion upon which scholarships are considered. Accepted applicants are expected to pay a commitment fee to be considered for a scholarship. You may be expected to pay the rest of the registration fees for the soccer season.
- CLUB reserves the right to determine the scholarship amount if the demand for scholarships is high. CISA also will take into account scholarship fund availability.
- Approval of any scholarship application does not automatically register a player into the program. The family/player is still responsible for completing the CISA registration form HERE. A commitment payment of $150.00 for competitive players and $75 for recreational players will need to be received prior to the registration deadlines and scholarship approval.
- Eligible programs include recreational and competitive team registration fees only.
- Team expenses, tournament expenses, and uniform expenses are separate expenses and are NOT covered by this scholarship.

When you accept your scholarship you are agreeing to and accepting these Terms and Conditions and agree to the requirements and conditions of your scholarship.
- Families are responsible for paying all player fees not covered by their scholarship. CISA offers needs based financial assistance for club fees based on the guidelines set forth in this application.
- CISA is not able to provide financial assistance for travel related fees except in extenuating circumstances determined and approved by the Program Director.
- CISA scholarships are subject to available resources. The number of awards offered each season may vary depending on the number of applicants who meet the scholarship requirements.
- CISA reserves the right to limit the maximum number of awards each season.
- The awarding of a scholarship is conditional upon the successful completion and receipt of the requested documentation.
- Families may be asked to provide current pay stubs and/or the most current tax return.
- Scholarships will be applied directly toward player fees and are not paid to families directly in the form of cash.
- Parents must agree to the terms of our parent agreement and players must agree to the terms of our player agreement.
- Scholarships are awarded based on established eligibility criteria. You must meet the eligibility requirements to receive your scholarship payment. The eligibility requirements for each scholarship are outlined on the first page of our scholarship application.
- Athletes who are awarded a scholarship may be given a discount code used at the time of registration or refunded for payments already made to CISA.
- Providing false or misleading information is an offense. If CISA knows or has reason to believe that you have provided false or misleading information, CISA may reassess your entitlement to the scholarship offered and you will be liable to re-pay to CISA the total sum of any scholarship awarded to you.
- By submitting this Scholarship application, you hereby authorize CISA and its employees, agents, contractors and those of its affiliates to release any information provided in or relating to your scholarship application to its representatives, who need to know such information for purposes of administering and promoting this program. CISA employees and agents will use the information only for purposes of evaluating scholarship applications.
- In the event that you are awarded a scholarship, you grant to CISA the right to use your name, likeness, and any direct quotes made by you in respect to the scholarship for publicity purposes without further compensation.
- Any personal information that you provide to CISA in relation to the scholarship program will be used solely for (1) the administration of the Program, (2) media releases and other public relations materials in relation to the promotion of the scholarship, and (3) in the aggregate and anonymous form for reporting and statistical purposes. CISA will not (i) sell your personal information to third parties, (ii) will store it in a secure manner, and (iii) will destroy it once it is no longer needed for the purposes of the Program. By completing the application, you consent to the collection, use and disclosure of your personal information for those purposes. If you are selected for a scholarship, then you also consent to the collection, use and disclosure of your personal information for the above purposes.
- CISA reserves the right to cancel the Scholarship program at any point without notice.
Eligibility Requirements:
- Proof of Active Free/reduced lunch: If the applicant qualifies for free or reduced lunch program at their school district, please provide a copy of the program acceptance letter.
- Proof of any active other state or federal assistance, please provide a copy of the program acceptance letter.
